We respectfully acknowledge the Traditional Custodians of the lands across Queensland and recognise their enduring connection to land, waters, skies, and culture. We pay our deepest respects to Elders past, present, and emerging, and honour their vital role in the ongoing journey of reconciliation, healing, and cultural preservation.

Nicole Ramsamy – Founder & Lead Clinician

Queensland’s first Indigenous Nurse Practitioner and second in Australia, Nicole has dedicated three decades to advancing First Nations health across remote regions. Her passion lies in community care, chronic disease prevention, monitoring and management, women’s health screening, child development assessments, continence assessments, behavioural supports, and supporting people with disability and their families.

Founded by a proud First Nations woman — an Eastern Kuku Yalanji and Boigu Island descendent — our company is built on over 30 years of nursing and midwifery experience in some of Australia’s most remote communities. Now based in Cairns, our founder has expanded her scope as a Nurse Practitioner and Endorsed Registered Midwife to serve NDIS participants across Far North Queensland.

Drawing from a deep understanding of the systemic barriers that First Nations peoples and remote communities face, we recognised a pressing need for healthcare and support services that are not only accessible, but also culturally safe and responsive. This lived experience inspired the creation of a service that goes beyond clinical care — extending into specialised NDIS support coordination tailored to the unique cultural, social, and geographic realities of our clients.
Our journey is guided by a commitment to equity, respect, and empowerment. We exist to bridge the healthcare divide, to honour cultural identity, and to deliver quality care and support — wherever it’s needed.

Our Mission: Empowering Lives with Compassionate Support

At Linkup2care, our mission is grounded in a deep commitment to delivering compassionate, culturally responsive, and comprehensive support to NDIS participants and their families—especially in rural, remote, and First Nations communities across Far North Queensland. We understand that every person has unique needs and circumstances, and we meet these with empathy, respect, and professionalism. Whether it’s through clinical nursing, specialised support coordination, or personalised support work, our care is always guided by the lived realities of our clients.

Promoting Dignity, Independence and Self-Determination

We believe that everyone deserves the opportunity to live a life of dignity, independence, and purpose. Our services are designed to support the whole person—physically, emotionally, culturally, and socially. From healthcare assessments and chronic disease support to help around the home, transport, community access, and companionship, we provide holistic, wraparound care tailored to each individual.

Culturally Safe, Inclusive, and Community-Led

Cultural safety is at the heart of everything we do. We honour the languages, values, and traditions of Aboriginal and Torres Strait Islander peoples and strive to create inclusive environments where clients feel heard, respected, and culturally understood.
Our team includes Indigenous practitioners and community members who bring deep knowledge of Country, kinship, and culture—ensuring care is not only appropriate but empowering.

Building Stronger Communities Through Partnership and Purpose

We know that stronger communities lead to better outcomes. Through collaboration with local organisations, engagement with Elders and community leaders, and active participation in outreach and education, we work to break down barriers and strengthen systems of support.

At Linkup2care, our mission is more than just words—it is our everyday practice. We are proud to be a trusted partner in the journeys of those we serve, walking alongside them towards greater health, resilience, and connection.
